LOCKER ROOM SCREENING INSTRUCTIONS

Background Screening 

Remember those background screening forms that you have to fill out every year? NO MORE FILLING THOSE OUT ON PAPER!!! Minnesota hockey is requiring all Coaches and board members to go online and fill it out electronically. THIS NEEDS TO BE DONE BEFORE THE SEASON/TRYOUTS BEGIN. EVEN IF YOU HAVE FILLED OUT A SCREENING FORM IN THE PAST IT NEEDS TO BE DONE EVERY YEAR. Please follow directions below. Click on link www.minnesotahockey.org

 Scroll Down and click on  box that says Screening.
 Scroll to bottom of page you will see a Coach Box and Other Positions.
Coaches need to click  "Coach", Managers and Locker Room Monitors click "Other Positions".
 

Log in, or create log in (If you register your kids with EHA, you can use the same username and password you used to register them just be sure and add yourself, do not click on your kids' names).

*If you do not have an account click on the right where it says "Sign up for account today" and be sure and write down your username and password as you will use that next year when you need to do the same thing.
*If you get a message saying you already have an account but you do not remember the username and password, click on the link on the login page that says "forgot username/forgot password". You must get the username first.

Follow through all of the forms and submit. WHEN PUTTING IN OUR ASSOCIATION, BE SURE AND TYPE IN THE FULL NAME "Edina" NOT EHA. THE DISTRICT IS 6.
